Why Dr. Shivani Gour is a messiah for us?

We introduce ourselves as Mrs. Romila Jha and Mr. Kamal Jha and we’re here to talk about Dr. Shivani Sachdev Gour. I am a proud mother of a baby girl who is playing with me right now. When I look at her the people I remember are my husband and Dr. Shivani. My husband stood beside me through thick and thin. Dr. Sachdev not only treated me, but she also gave me strong support.

Complications before my delivery and meeting my savior

I experienced several complications before and after my pregnancy. I had a cyst a year before I conceived. I went to another gynecologist and asked for advice. She told me that nothing would go wrong if she delivered. Unfortunately, that was not the case and I had too much bleeding during delivery and I lost my first baby. It took us quite a long time to come out of the shock. Then we decided to consult a doctor after I become pregnant once again. My husband was also of the same opinion. Finally one of my relatives suggested the name of Dr. Shivani Sachdev Gour.

The day I met the good doctor

We went to meet the doctor at her Delhi chamber with a tottering heart and a glimmer of hope. She listened to us very patiently, studied the reports minutely, and then took me for a checkup. She suggested that we should go for IVF or C section as normal delivery would not be possible for me. Dr. Shivani Sachdev also gave some medications and a few routine clinical exams.

The treatment and delivery

We did our bit of research about IVF and learned that the success rate was low for this treatment. I am not ashamed to admit we also consulted other clinics too who promised us 90% positive chances of delivery. Finally, we went back to Dr. Shivani and opted for IVF. She didn’t show us figures nor did she promise us anything. She told us to hold on to our faith and trust her. After a long treatment that went for a few months, finally, I delivered!

Finally, my baby in my hands and the doctor becomes our savior

I couldn’t believe my eyes when the nurse gave my life in my hands. Dr. Shivani reviewed my reports and told me I was fine. Words are secondary to the gratitude I have for this doctor. She is a messiah for us and is truly a humanitarian doctor. Thank you, Madam!

How Does Surrogacy in India Influence The Decision of Couple?

Marriage is an institution where two individuals get tied in a relationship mutually. When the couple plans to take a responsibility of becoming parents, the period seems to be very golden. But, for few of the couples, the moment of truth shocks them. They realize that the alternative to pregnancy is what can retain their happiness. This is simply due to some of the issues due to which the woman cannot conceive or there may be some complications in any of the two individuals. In either of the cases, the only solution is to make the utmost use of surrogacy in India. Basically, with the help of surrogate mother, the couple can cherish the moment of becoming proud parents of the child.

But, there are certain laws associated with surrogacy in the Indian law. It is necessary to abide by these laws while looking for a surrogate mother or signing the surrogacy agreement. This will keep both the surrogate mother and the couple on the safer side and experience the magic of this scientific yet medical process. Nevertheless, one has to follow the laws related to surrogacy in India prior to accepting this medical way conception. One of the major laws of surrogacy in the Indian law states that no form of trafficking is acceptable, wherein, the person is being administered with narcotics or any such stuff. This is a sheer form of injustice and unlawful activity from the point of Law.

Besides, the lady who agrees to become a surrogate mother should not be tortured or injured as a consequence of trafficking. This indeed is unlawful act and prohibited by the Indian law during the process of surrogacy. According to the amended laws for surrogacy in India, if any person is found guilty as a result of practicing trafficking on the pretext of trafficking gets punishment or rigorous imprisonment. The Indian law is very particular about these amendments because even surrogate mothers have a right to freedom and they should not at all be manipulated by anyone. Apart from this, in no way, should anybody administer chemical substance in the body of surrogate mother to attain sexual maturity! This is a punishable office, as directed by the Indian law.

Over the years, surrogacy has become acceptable in different parts of India, but, people need to be informed about its ethical approach. In simple words, there is a list of offences and penalties which are listed in the stream of law with respect to surrogacy in India. One of the major offences includes the illegal migration of a person as a part of surrogacy. This is not permitted in the Indian law and often counts as the activity under human trafficking. Furthermore, it should be remembered surrogacy in Indian society is just a way to assist the couples lead a happy family. In no way, should people misuse this medical boon because the laws related to this condition are very strict.

Lastly, the couple that wants to bear a child through surrogacy should definitely abide by the Indian rules! In this way, even the surrogate mother will neither be uncomfortable nor will the couple have to witness the law offenses or penalties!

Preimplantation Genetic Diagnosis

Preimplantation genetic diagnosis (PGD) is a procedure which involves checking the genes or chromosomes of the embryos prior to implantation to help identify genetic defects within embryos. This helps to prevent certain genetic diseases or disorders from being passed on to the child before it is transferred. The embryos need to be tested in a lab prior to implantation into the womb, even if the partners have no fertility problems. Embryos which have been tested for genetic defects and are free of the condition will be transferred into womb and allows the identification and transfer of embryos free from aneuploidies to the mother. It also significantly increases pregnancy rates per transfer and reduces the time needed to achieve pregnancy. The embryos used in this procedure are usually created during the process of in vitro fertilization (IVF). With PGD testing, embryos created through IVF procedure are cultured in a laboratory for three days until they reach the 8-cell stage. At this stage a blastomere biopsy is done in which one or two of the blastomeres are removed by inserting a micropipette through the zona pellucida which surrounds the embryo.

It is significantly helpful when one or both genetic parents has a known genetic abnormality and testing is performed on an embryo to determine if it also carries a genetic abnormality to the child.PGD provides an alternative to current postconception diagnostic procedures (i.e. chorionic villus sampling or amniocentesis) which are usually followed by the difficult decision of abortion if results are unfavorable. It is currently the only options available for avoiding a high risk of having a child affected with a genetic defect prior to implantation. It is consisted two steps, extraction of one or two cells from an IVF-produced embryo and application of the PGD test. PGD is important for doctor and embryology because it has advanced IVF results and allowed couples more opportunities to deliver a healthy child free from any potential genetic defects.

PGD was originally developed in the early 1980s as an alternative to post-implantation prenatal testing. The only other forms of prenatal diagnosis available involved chorionic villus sampling (CVS) and amniocentesis before this technology. Both of these technologies imply the examination of embryos during later stages of fetus development, when the embryo is already developing inside the mother womb. Couples who choose any of these tests must decide whether or not go for abortion if test results suggest positive for a particular genetic disease. In the initial stage of its development, PGD was used as a form of gender selection to avoid having a child with a sex-linked disease. Couples using PGD often choose to have only girls since males are prone to be affected by X-linked genetic disorders. PGD rapidly came to be used for three main groups of inherited disease, single-gene mutations such as cystic fibrosis and sickle cell anemia, chromosomal abnormalities such as Down syndrome and sex-linked disorders such as hemophilia.

Although PGD was originally thought to be a mechanism to dispose embryos affected by a particular genetic disease. Some couples have used PGD as a means of creating siblings who can provide transplants for their older siblings.

SCI IVF Hospital and Multi-specialty Center is one of the best places for PGD procedures. This procedure is performed under the supervision of Dr Shivani Sachdev Gour in this hospital.

Common Types of Pneumonia during Pregnancy

Depending on the nature of its transmission, pneumonia can be classified into many categories. The general forms of pneumonia significantly visible in pregnant women are given below:

Bacterial Pneumonia:

Bacterial pneumonia is the most common pneumonia in both pregnant and non-pregnant women. Bacterial pneumonia make pregnancy more complicated 1 in 600 pregnancy cases. This type of pneumonia may be arising due to more incidences of women with chronic diseases becoming pregnant. Bacteria normally reach the lungs by aspiration or inhalation and originates from a bacteria carried by someone. The bacteria that cause this include streptococcus pneumonia, mycoplasma pneumonia and haemophilus influenza. These bacteria normally live in the upper respiratory tract. These bacteria can be known as “atypical” because pneumonia caused by these organisms might have somewhat different symptoms, respond to different antibiotics than the typical bacteria that cause pneumonia appears different on a chest X-ray. Even though these infections are not uncommon and are called “atypical”. Bacterial pneumonia can develop after you’ve had a viral cold or the flu or occur on its own. The people those at greatest risk for bacterial pneumonia include people who have weakened immune systems, people with respiratory disease or viral infection and people recovering from surgery. Symptoms include chills, fever, fast breathing, and shortness of breath, fatigue and chest pain.

Viral Pneumonia

Viruses are the second most common cause of pneumonia. Viruses that infect the upper respiratory tract may also responsible for pneumonia. The same viruses that bring on colds and flu cause the viral pneumonia. The most common viral pneumonia is influenza and varicella. The influenza virus is the most common reason of viral pneumonia in adults. Viral pneumonia caused by the influenza virus can be dangerous and sometimes life threatening. Influenza more aggressively spread during pregnancy than bacterial pneumonia. With a mortality rate of 10 percent in pregnant and also non pregnant women, varicella pneumonia is the more serious condition of the Viral Pneumonia. Varicella can also be increase the risk of preterm delivery.

Viral pneumonia can become more serious if the pregnant woman also contracts bacterial pneumonia.  This combination often leads to death. Clinically viral pneumonia is somewhat hard to differentiate from bacterial pneumonia because symptoms are similar in both cases. Most viral pneumonias are not severe and last only a shorter time than bacterial pneumonia in most patients. The virus approaches the lungs and multiplies rapidly. This pneumonia is most serious in pregnant women and people who have pre-existing heart or lung disease. The risks of viral pneumonia further complicated by a secondary invasion of bacteria, exhibits all the typical symptoms of bacterial pneumonia.

Fungal Pneumonia

This type of pneumonia is less common cause of pneumonia, and it is primarily due to a fungal called ‘coccidioidomycosis’ and it normally occur in patients who are severely immunocompromised, especially in those with AIDS. The fungal pneumonia spreads by breathing in tiny particles called fungal spores. It can affect immunity and stimulate effect on progesterone in the affected women. This pneumonia has a mortality rate of 25 percent. It symptoms include, fever, dry cough, anorexia and dyspnea. People in certain activities such as farmers, landscapers and gardeners are more likely to come into contact with such pneumonia.

Health Risks of Primary Ovarian Insufficiency (POI)

Health Risks of Primary Ovarian Insufficiency (POI)

As women with POI have lower levels of certain hormones, several health problems are more common than in women without POI, including:

  • Infertility: Women suffering from POI, generally unable to get pregnant naturally. They may carry a pregnancy but, in most cases, adonor eggsare required.
  • Heart disease: Low estrogen level early in life can affect the muscles lining the arteries and can increase the formation of cholesterol in the arteries. These factors can increase the risk of atherosclerosis (hardening of the arteries) which increases the risk of heart attacks further.
  • Anxiety and depression: Hormonal changes caused by POI can result to anxiety or lead to depression. Many women with POI may feel sadness about their unexpected loss of ovarian function. Women with POI should discusses their feelings with their family members, medical provider and may need to discuss with a qualified therapist.
  • Dry eye syndrome and eye surface disease: Some women with POI can have one of these eye diseases. Both diseases can cause discomfort and may lead to blurred vision. These conditions can cause permanent eye damage, if not treated seriously.
  • Osteoporosis: The hormone estrogen required to keep bones strong. Women with POI often develop osteoporosis without enough estrogen. It is a bone disease that increase the risk of weak bone, brittle bones or bone loss and fractures.
  • Low thyroid function: This health problem also is known as hypothyroidism. The thyroid is a gland that produces hormones that control body’s metabolism and energy level. Low levels thyroid hormones can affect your metabolism and can result in very low energy, mental flatness and other similar symptoms.

How is POI diagnosed?

if you are younger than 40 and have irregular periods or stopped having irregular ones, immediately consult with your doctor to find the root causes of the ongoing problem. To diagnose POI, your health care provider may perform:

  • A medical history: Your health care provider may ask whether you have relatives with POI or other health conditions such as an autoimmune disease, endocrine problems or a neurological condition.
  • A pregnancy test: A test can be done to make sure that you are not pregnant.
  • A physical examination: To look for signs of other disorders which could be causing your symptoms.
  • A pelvic ultrasound: An ultrasound may be used to check whether or not the ovaries are enlarged or have multiple follicles.
  • Blood tests: A blood test has been done to check for certain hormone levels. With this test your health care provider (HCP) can find out if your ovaries are working properly or not and to check the level of FSH (Follicle Stimulating Hormone) in your blood. This blood test can be repeated before the diagnosis can be made.High levels of FSH usually indicate presence of POI. Other hormones including an “AMH” (Anti-Mullerian Hormone) can be evaluated and genetic tests may be done to check if there is a medical reason for the Primary Ovarian Insufficiency (POI). Other blood test can be done to do a chromosome analysis. A chromosome is the part of a cell that contains genetic information.

Some specific tests should also be performed to check for autoimmune conditions or possible genetic that might be related to the ovarian insufficiency.
